Following the loss of a computer memory stick, the personal details of thousands of criminals are now ripe for abuse. Is this an unfortunate lapse or a massive sign of failure?

This is not the first time the Government has mislaid the personal details of its citizens. BBC analysis reports that more than 4 million sensitive records have been lost by government agencies over the last year.
